Custodian of Records Notice

Goldfinch Counseling LCSW LLC has ceased operations. Existing staff and clients have been brought into our sister company, Engage Psychotherapy LCSW PLLC. This notice is to provide previous Goldfinch Counseling clients with important information regarding the security and future storage of your medical records.

In compliance with New Jersey state laws and the regulations of the State Board of Social Work Examiners, your clinical records must be maintained and kept accessible for a period of seven years from the date of your last visit (or, for minors, seven years past the age of majority).

Designation of Custodian of Records We have designated a specific entity to serve as the "Custodian of Records." This ensures that your private health information remains secure and that you have a point of contact should you need to access your file or transfer it to a new provider in the future.

How to Request Your Records If you or your new provider require a copy of your clinical records in the future, please contact Engage Psychotherapy directly. Standard authorization procedures will be followed to ensure the protection of your confidentiality.
